Birth Registration
Birth Registration
For
birth registration of children born to Pakistani parents in Uzbekistan,
following are the requirements:
- Filled in Form S which can be downloaded from https://dgip.gov.pk/downloads/form_s.pdf or can be obtained from the Embassy.
- Original and copy of Birth Certificate from Civil Registry Office of Uzbekistan. The Birth Certificate must be translated in English from Notarius (Notary).
- Two recent passport size photographs of the child with white or blue background.
- Copy of the Passport & CNIC/NICOP of Father and Mother (Mother’s CNIC/NICOP must have husband’s name on it)
- Registration fee of US$. 10/- each.
